Fountain View at Logan Square, formerly known as The Watermark, is home to a comprehensive continuum of care that includes a skilled nursing facility, and a full Continuing Care Retirement Community with both enhanced senior living and independent living apartments. I went to Logan Square after a fall at home. I broke my hip and had to have extensive surgery. I was not strong enough to return to my home and after many conversations with family and friends, I chose Logan Square. I am so happy I did because everyone was so nice and caring. The physical therapist had me back on my feet in no time. There were days I didn’t feel like getting out of bed but with the help and kindness of the staff, they really motivated me to push myself. I can’t believe I get to go home tomorrow and I can’t wait to make my family dinner again!
The food was excellent, and I thank the chefs very much. My therapist Kevin is a very concerned caregiver that makes you want to promote healing. I couldn’t do it without you all. The nursing and aid team was wonderful. Karen, the housekeeper on the subacute wing was more than an inspiration to me. The staff is always willing to help, and I would recommend Laurel Brook to anyone.
My stay here at Logan Square has been wonderful! Tommy was the first person to greet me when I arrived and he gave me such a warm welcome. The rest of the staff has been great too, from the nurses to the therapy staff… they all work so hard! I was pretty sick in the hospital and when I arrived at Logan Square, I was very weak. It took me some time but I am finally feeling better and am excited to get home to see my wife and my dogs. I would 100% recommend my family and friends to Logan Square if they ever need rehab.
Thank you to the administrator Sydney for your involvement and concern. The nurse Annette and Kia were wonderful in working with me and making me comfortable. Donna Ray a frequent aide was wonderful. Housekeeping that came in and cleaned the room frequently were friendly and engaging. Most importantly your therapy department is top shelf. Finally, Brianah and her colleague were always available and guided me well. So again, thank you to you Sydney once you got involved my entire experience has become remarkably positive thank you for listening and addressing my concerns with a deep bow of gratitude.
Rehab is awesome. Craig has a great staff behind him. Brian, Jesse, Dean, Adam, Annette and Amy are all so kind and welcoming. Crystal, unit clerk, is great as well. She makes sure I’m accommodated with my appointments. Julie made sure all my meds were given to me in a timely manner. The aides Kareema, and Deb always give excellent conversation when giving me care. Tameeka is very caring and a great ear for listening. The rehab team is always following through on my concerns. Their mission was to care for me and help me get back to my life at home. It was a process and I’m glad I made the decision to admit here at Logan Square. Thank you for all your kind words.
