Welcome to Logan Square Rehabilitation & Healthcare Center, a warm and welcoming skilled nursing center located in the heart of Center City in downtown Philadelphia. As part of the vibrant Fountain View campus, our center features an age-in-place continuum of care that includes the best in subacute rehabilitative and long-term care. We are excited to infuse our center with our progressive rehab approach, and our onsite Dialysis Unit will be reopening in the very near future.
Our soaring, landmark building is an integral part of the cultural and historic downtown and offers scenic views of the nearby fountain in conjunction with the highest level of clinical care. We are proud to be a preferred partner of Jefferson Health System and have some of the area’s best healthcare providers on our staff! Our caregivers strive to enrich and enhance the quality of life of each individual, and are dedicated to meeting all their physical, emotional, and spiritual needs.
Our Subacute Wing
Our subacute wing—dedicated to the post-hospitalization care of individuals recovering from joint replacement, cardiac, pulmonary, neurological, and other acute medical conditions—features private and semi-private suites, physiatry-driven rehab programming, state-of-the-art equipment, and daily therapy. Our rehab program is recognized for successfully reducing hospital readmissions and restoring physical function to individuals recovering from surgery or illness.
